PB-1 Form for ATU Members
ATU members may use this form to get a day off with pay if they get a preventive wellness exam and biometric screening. ATU members may also use this form in order to get a preventive exam/test at the VA or a mobile screening unit.
Completed forms must be submitted to the Trust Office.
Cigna Wellness Screening Form
Use this form to get credit for a biometric screening as part of the Health Assessment Healthy Activity. Completed forms must be submitted to Cigna.
